Photo, Video & Media Release Consent

I, the undersigned, hereby grant permission to Playtime Event Sitters to photograph, record, and/or videotape my child(ren) during services and events. I understand and agree that these photographs, audio recordings, and video recordings may be used by Playtime Event Sitters for lawful purposes, including but not limited to print and digital marketing materials, website content, social media platforms, presentations, and other promotional materials. I acknowledge that all images and recordings are the sole property of Playtime Event Sitters and may be used without further approval. I understand that neither I nor my child(ren) will receive any compensation for the use of these images or recordings. By signing below, I confirm that I am the parent or legal guardian of the child(ren) listed and that I voluntarily grant this consent.*

Waiver

I, the undersigned (parent or guardian) consent to Playtime Event Sitters’ caregivers to care for my children at events where they provide childcare. I understand that there is a COVID-19 pandemic and Playtime Event Sitters will follow every effort to prevent the spread by following CDC guidelines and I the undersigned will do so as well and will inform Playtime Sitters if myself, those who I have contact with or my children have any symptoms within the last 14 days before leaving them in the childcare room. Further, I do release and hold harmless Playtime Event Sitters, its directors, officers, employees, representatives, advertisers, partners, clients, contractors and its successors, for any and all claims, suits or action arising out of in connection with exposure, infection, and/or spread of COVID-19 related to utilizing Playtime Event Sitters' service and/or an accident whereby my child is injured or suffers any illness regardless of if the injuries are caused by negligence. I authorize in any emergency for Playtime Event Sitters to take what action is necessary to receive medical attention for my children. I further agree to indemnify and/or reimburse Playtime Event Sitters, its employees, agents, officers, contractors and directors from any and all expense, cost, claims, judgments or executions which may occur due to any accident or event which gives rise to injury or illness to my children.*
